ABSTRACT:
Pregnancy is a significant physiological and psychological transition in a woman’s life. Primigravida women commonly experience stress, anxiety, sleep disturbances, emotional instability, and fear related to childbirth and fetal wellbeing. These maternal psychological disturbances can adversely influence fetal growth and development. Complementary and non-pharmacological therapies such as lullaby therapy and music-based interventions have recently gained attention in maternal healthcare due to their calming and therapeutic effects. This systematic review aimed to evaluate the influence of lullaby therapy on maternal health and fetal development among primigravida women. Objectives to assess the effectiveness of lullaby therapy on maternal psychological and physiological health. To evaluate the impact of lullaby therapy on fetal development and fetal wellbeing. To identify the benefits of lullaby therapy as a complementary intervention during pregnancy. To analyze the evidence related to maternal and fetal outcomes associated with prenatal lullaby therapy. A systematic review was conducted using articles published between 2020 and 2026 from Pub Med, Google Scholar, Scopus, ScienceDirect, and Web of Science databases. Keywords used included “lullaby therapy,” “music therapy during pregnancy,” “maternal wellbeing,” “fetal development,” “prenatal relaxation therapy,” and “primigravida women.” Studies involving antenatal mothers, especially primigravida women, were included. Research articles, systematic reviews, randomized controlled trials, quasi-experimental studies, and observational studies were reviewed. The reviewed studies consistently demonstrated that lullaby therapy significantly reduced maternal stress, anxiety, depression, and sleep disturbances. Physiological improvements such as reduced pulse rate, blood pressure stabilization, and improved relaxation were also reported. Positive fetal outcomes included improved fetal heart rate variability, increased fetal movement, enhanced maternal–fetal attachment, and better neurobehavioral responses. Studies indicated that regular exposure to soothing lullaby music positively influenced maternal emotional wellbeing and fetal development. Lullaby therapy was found to be an effective, safe, economical, and non-invasive complementary intervention that improved maternal psychological wellbeing and supported positive fetal developmental outcomes among primigravida women. Integration of lullaby therapy into routine antenatal care may enhance holistic maternal and fetal health.

INTRODUCTION:
Pregnancy is one of the most important phases in a woman’s life and involves profound physical, emotional, hormonal, and psychological changes. Primigravida women, who experience pregnancy for the first time, often encounter fear, anxiety, uncertainty, stress, and emotional instability related to childbirth and fetal wellbeing. Maternal stress during pregnancy has been associated with adverse maternal and fetal outcomes including hypertension, sleep disturbances, depression, poor maternal adaptation, preterm labor, low birth weight, and altered fetal neurodevelopment.
Maternal psychological wellbeing is increasingly recognized as an essential component of antenatal care. Along with medical management, complementary and alternative therapies have gained importance in improving maternal comfort and promoting fetal wellbeing. Among these therapies, music therapy and lullaby therapy are widely studied due to their relaxing and therapeutic effects.
Lullaby therapy refers to the use of soothing rhythmic music or calming songs designed to induce relaxation, emotional comfort, and psychological stability. Listening to lullabies stimulates the parasympathetic nervous system, reduces cortisol secretion, stabilizes maternal vital signs, and promotes emotional balance. Relaxing music also influences fetal behavior by regulating fetal movement and fetal heart rate patterns.
The fetus begins responding to sound stimuli during the second trimester, and repeated exposure to soothing sounds may positively influence fetal neurodevelopment and auditory stimulation. Music heard during pregnancy can create an emotional connection between the mother and fetus, thereby strengthening maternal–fetal attachment.
Recent studies have shown that lullaby therapy effectively reduces maternal stress and anxiety while promoting relaxation, improved sleep quality, and emotional wellbeing. In addition, fetal responses such as improved fetal heart rate variability, increased fetal movement, and better fetal neurobehavioral development have been observed.
Despite increasing evidence, the available findings remain scattered across different populations and healthcare settings. Therefore, this systematic review was undertaken to synthesize current evidence regarding the influence of lullaby therapy on maternal health and fetal development among primigravida women.
To evaluate the influence of lullaby therapy on maternal health and fetal development among primigravida women.
1. To assess the effectiveness of lullaby therapy on maternal psychological health.
2. To evaluate the effect of lullaby therapy on maternal physiological wellbeing.
3. To determine the influence of lullaby therapy on fetal development and fetal wellbeing.
4. To identify maternal and fetal benefits associated with prenatal lullaby therapy.
5. To analyze the role of lullaby therapy as a complementary intervention during pregnancy.
METHODOLOGY:
This systematic review was conducted to evaluate the influence of lullaby therapy on maternal health and fetal development among primigravida women. Relevant literature was collected from electronic databases including PubMed, Google Scholar, Scopus, ScienceDirect, Web of Science, and Cochrane Library. A comprehensive literature search was carried out using keywords such as “lullaby therapy,” “music therapy during pregnancy,” “prenatal music intervention,” “maternal wellbeing,” “maternal stress,” “fetal development,” “fetal wellbeing,” “primigravida women,” and “prenatal auditory stimulation.” Boolean operators such as “AND” and “OR” were used to refine the search process and retrieve appropriate studies related to the topic. Studies published between 2020 and 2026 were included in the review. Randomized controlled trials, quasi-experimental studies, observational studies, systematic reviews, and review articles conducted among antenatal mothers, particularly primigravida women, were considered for analysis. Studies involving lullaby therapy or music-based prenatal interventions and reporting maternal or fetal outcomes were included, while studies unrelated to pregnancy, non-English articles, conference abstracts, editorials, unpublished reports, and duplicate articles were excluded. Initially, articles were screened based on title and abstract, and relevant full-text articles were further evaluated according to the inclusion and exclusion criteria. The selected studies were critically reviewed and synthesized narratively under themes related to maternal psychological wellbeing, physiological health, maternal–fetal attachment, fetal development, and fetal responses. Ethical principles related to plagiarism prevention and proper citation of references were strictly maintained throughout the review process.
REVIEW OF LITERATURE:
Chang Sung et al., (2021) conducted a study on the effect of lullaby music therapy on maternal stress and fetal response among primigravida women.1 The objective of the study was to evaluate the effectiveness of lullaby therapy on reducing maternal anxiety and improving fetal physiological responses during pregnancy. The population included primigravida antenatal mothers in the third trimester. The setting was antenatal clinics of a tertiary care hospital in South Korea. The tool used included the State Trait Anxiety Inventory, maternal pulse rate assessment, and fetal heart rate monitoring. Convenience sampling was used among 60 antenatal mothers. The results showed that mothers receiving lullaby therapy had significantly reduced anxiety levels and improved fetal heart rate patterns. The study concluded that lullaby therapy effectively promoted maternal relaxation and fetal wellbeing. The researchers recommended the incorporation of lullaby therapy into routine antenatal care.
Garcia et al., (2020) conducted a study on the influence of prenatal music therapy on maternal emotional health and fetal movement.2 The objective was to determine the effects of soothing lullaby music on maternal emotional status and fetal activity. The population consisted of pregnant primigravida women between 28 and 36 weeks of gestation. The setting was community maternity centers in Spain. The tools used were the Perceived Stress Scale, fetal kick count chart, and maternal interview schedule. Purposive sampling was adopted among 80 antenatal mothers. The findings revealed lower stress scores and improved emotional comfort among mothers exposed to lullaby therapy. Increased fetal movement was also observed. The study concluded that prenatal lullaby therapy positively influenced maternal emotional wellbeing and fetal behavioral responses. The researchers recommended regular music-based relaxation sessions during antenatal visits.
Liu et al., (2022) conducted a study on the effectiveness of lullaby intervention on maternal sleep quality and fetal wellbeing.3 The objective was to assess whether lullaby therapy improved sleep quality among primigravida mothers and enhanced fetal wellbeing. The population included primigravida mothers in the third trimester. The setting was a maternity hospital in China. The tools used were the Pittsburgh Sleep Quality Index, fetal heart rate monitoring, and sleep diary. Random sampling was used among 100 antenatal mothers. The findings showed improved sleep quality, decreased nighttime awakenings, and better fetal heart rate variability in the intervention group. The study concluded that lullaby therapy improved maternal sleep and supported fetal neurological stability. The researchers recommended integrating lullaby therapy into prenatal nursing care.
Hernandez et al., (2021) conducted a study on the impact of music-based relaxation therapy on maternal anxiety and fetal development.4 The objective was to identify the effect of lullaby music on maternal anxiety reduction and fetal growth indicators. The population included primigravida antenatal mothers aged 20–30 years. The setting was obstetric units of selected hospitals in Mexico. The tools used included the Hamilton Anxiety Rating Scale, ultrasound fetal growth assessment, and maternal blood pressure monitoring. Simple random sampling was used among 70 participants. The results revealed decreased anxiety levels and improved fetal growth measurements in mothers receiving lullaby therapy. The study concluded that lullaby therapy positively influenced maternal psychological health and fetal growth outcomes. The researchers recommended longitudinal studies to evaluate long-term developmental effects.
Garciaet al., (2022) conducted a review on prenatal auditory stimulation and fetal neurodevelopment. [5]The objective was to evaluate the role of prenatal sound stimulation in fetal neurological development. The population included pregnant women exposed to prenatal music interventions. The setting included multiple antenatal healthcare centers and research institutions. The tools involved fetal neurobehavioral assessments and auditory stimulation measures. Studies were selected through systematic literature review methods. The findings showed that prenatal music exposure positively influenced fetal movement, fetal heart rate variability, and early auditory memory. The study concluded that prenatal auditory stimulation contributed to fetal neurodevelopment. The researchers recommended further neurodevelopmental follow-up studies.
Sharma et al., (2023) conducted a study on complementary therapies in antenatal care and the effectiveness of music therapy on maternal psychological outcomes.6 The objective was to assess the impact of music therapy on anxiety and emotional wellbeing during pregnancy. The population included antenatal mothers receiving prenatal care. The setting was community health centers in India. The tools included the Anxiety Rating Scale and maternal wellbeing assessment checklist. Purposive sampling was used among 85 pregnant women. The findings showed reduced anxiety and improved emotional stability among women who received music therapy. The study concluded that music therapy was an effective complementary intervention during pregnancy. The researchers recommended including music therapy in maternal health promotion programs.
Hatem et al.,(2020) conducted a review study on the therapeutic effects of music during pregnancy on maternal and fetal outcomes.7 The objective of the study was to synthesize evidence regarding prenatal music interventions. The population included pregnant women from various research studies. The setting involved hospital-based and community-based studies. The tools used included maternal anxiety scales, fetal monitoring techniques, and relaxation assessments. Systematic review methods were used to select studies. The findings demonstrated positive effects of music therapy on maternal stress reduction, emotional wellbeing, and fetal physiological stability. The study concluded that music therapy was beneficial and safe during pregnancy. The researchers recommended large-scale randomized trials.
Patel et al.,(2023) conducted a study on the effect of antenatal lullaby therapy on maternal stress reduction and maternal-fetal attachment.8 The objective of the study was to evaluate the impact of lullaby listening on maternal stress and maternal-fetal attachment. The population included primigravida women attending antenatal clinics. The setting was selected maternity hospitals in India. The tools used included the Prenatal Attachment Inventory and Depression Anxiety Stress Scale. Non-probability purposive sampling was used among 90 antenatal mothers. The results showed improved maternal-fetal attachment scores and reduced stress levels after four weeks of lullaby therapy. The study concluded that lullaby therapy enhanced emotional bonding and psychological adaptation during pregnancy. The researchers recommended nurse-led music therapy interventions in antenatal care services.
Johnson et al., (2020) conducted a study on the role of lullaby therapy in improving maternal wellbeing and fetal response.9 The objective of the study was to assess the influence of lullaby therapy on maternal wellbeing and fetal responsiveness. The population consisted of third trimester primigravida women. The setting was prenatal wellness clinics in the United States. The tools used included a Visual Analogue Scale for relaxation, fetal movement chart, and maternal blood pressure assessment. Cluster sampling was used among 75 pregnant women. The results demonstrated higher relaxation scores and reduced blood pressure among mothers who received lullaby therapy. Fetuses also showed more regular movement patterns. The study concluded that lullaby therapy was an effective non-pharmacological intervention for maternal relaxation and fetal stimulation. The researchers recommended educating pregnant women regarding prenatal music therapy.
RESULTS AND DISCUSSION:
The reviewed studies consistently demonstrated that lullaby therapy effectively reduced maternal stress, anxiety, fear, and emotional instability among primigravida women. Pregnancy-related anxiety is common among first-time mothers due to uncertainty regarding labor, fetal wellbeing, and maternal role adaptation. Listening to soothing lullaby music promoted relaxation and emotional calmness by reducing sympathetic nervous system activity and lowering stress hormone secretion. Several studies reported statistically significant reductions in anxiety scores among mothers receiving prenatal music interventions. Emotional relaxation improved maternal coping ability and psychological adaptation during pregnancy.
Lullaby therapy positively influenced maternal physiological wellbeing. The reviewed studies showed stabilization of blood pressure, reduced pulse rate, improved respiratory pattern, and enhanced sleep quality. Relaxing music promoted parasympathetic nervous system activation, resulting in physiological calmness. Improved sleep quality was another major finding among mothers exposed to lullaby therapy. Pregnancy-related discomfort and anxiety often contribute to sleep disturbances. Music therapy reduced nighttime awakenings and improved overall sleep satisfaction.
Maternal-fetal attachment refers to the emotional bond that develops between the mother and fetus during pregnancy. Studies included in the review reported improved maternal-fetal attachment among mothers who practiced lullaby therapy regularly. The emotional connection created during music listening sessions enhanced maternal affection, emotional communication, and positive maternal perception toward the unborn baby. Strengthening maternal-fetal attachment may positively influence maternal caregiving behavior after childbirth.
The reviewed studies demonstrated beneficial fetal responses associated with lullaby therapy. Improved fetal heart rate variability, increased fetal movement, and enhanced fetal behavioral responses were commonly reported. The fetus begins responding to sound stimuli during intrauterine life. Repeated exposure to soothing auditory stimulation may contribute to fetal sensory and neurological development. Music therapy may enhance fetal auditory memory and neurobehavioral responses. Studies involving fetal monitoring indicated that lullaby therapy promoted physiological stability and relaxation in the fetus. Some studies also suggested improved fetal growth measurements and enhanced fetal neurological organization.
Lullaby therapy was identified as a safe, economical, simple, and non-invasive intervention suitable for antenatal care. Unlike pharmacological therapies, music-based interventions do not produce adverse side effects and can be easily practiced at home. Healthcare professionals can incorporate lullaby therapy into antenatal education programs to improve maternal comfort and emotional wellbeing.
Despite positive findings, several limitations were identified in the reviewed literature. Differences in study design, sample size, duration of intervention, and measurement tools created variability among findings. Many studies involved small sample sizes and short follow-up periods. Further randomized controlled trials with larger populations and long-term follow-up are necessary to establish stronger evidence regarding the effectiveness of lullaby therapy on maternal and fetal outcomes.
NURSING IMPLICATIONS:
Large-scale randomized controlled trials should be conducted to strengthen evidence regarding lullaby therapy. Longitudinal studies should evaluate the long-term neurodevelopmental outcomes of prenatal music exposure. Comparative studies should be conducted between different types of music interventions. Antenatal education programs should include awareness regarding lullaby therapy. Nurse-led prenatal relaxation programs should be encouraged in maternal healthcare settings.
The present systematic review demonstrated that lullaby therapy positively influenced maternal health and fetal development among primigravida women. The reviewed evidence indicated that prenatal lullaby therapy effectively reduced maternal stress, anxiety, emotional disturbances, and sleep problems while improving relaxation and emotional wellbeing. Positive fetal outcomes including improved fetal movement, stabilized fetal heart rate variability, enhanced neurobehavioral responses, and better maternal-fetal attachment were also observed. Lullaby therapy was identified as a safe, economical, feasible, and non-invasive complementary intervention suitable for antenatal care. The findings support the incorporation of lullaby therapy into holistic maternal healthcare practices to promote psychological wellbeing and positive fetal development. Further high-quality research is necessary to strengthen evidence and evaluate long-term outcomes.
